Below are a list of required (*) and optional forms that Fellows should use, as appropriate.
Successfully completing the two-year program requires not only completing the PD sessions, but also completing your identity-inclusive project (course, module, or activity). Submit evidence of your deliverable implementation via the Deliverable Documentation Form
The 3C Survey allows for assessing the impact of your project via the pre/post-survey that measures participants cultural competence. Information on accessing and distributing the survey is available on the 3C Survey page
As new courses, modules, and activities are introduced globally by 3C Fellows, it's important that we understand the impact of these from the participants. Every 3C Fellow implementing an identity-inclusive project (course, module, activity), must have participants complete our Identity-Inclusive Post-Activity Feedback (also available at https://bit.ly/identity-feedback OR https://duke.qualtrics.com/jfe/form/SV_50ziImidtynLItE for short/full link) to help understand the impact of these efforts over time. The results are shared with each Fellow, for your own use in dissemination, annual reports, and more.
As part of the Alliance for Identity-Inclusive Computing Education (AiiCE), The DO-IT/AccessComputing team offers complimentary audits of project materials (e.g., website, syllabus, etc.) to ensure all activities are accessible for people with disabilities. Interested Fellows should complete the Accessibility Audit Request Form, and a member of the DO-IT/AccessComputing team will follow up with you.
If you change organizations, email, or other information, please complete the Contact Information Update Form so that we may remain in contact with you.
In an effort to help us report and celebrate your accomplishments as a result of the 3C Fellows Program, please complete the 3C Fellows Accomplishments Form to inform us of any achievements re: your deliverables, publications/presentations professional success, and more. This helps us report the impact/sustainability of the program to funding agencies.
